Filter to what you need
- •state: US state/district/territory codes, applied server-side, e.g. ["NY","NJ"]

- •keyword: case-insensitive substring across full name and NPI
- •zipPrefix: 1-9 digit prefix match against the ZIP

- •currentlyOptedOutOnly: keep only records whose end date has not passed

Default run with no input returns the first 100 records of the current release. maxResults ranges 1 to 20,000 and sets the bill directly. Only the state filter runs server-side; every other filter reads more of the dataset locally, up to about two minutes for a nationwide scan at the CMS-mandated 10-second crawl delay.

57,530 physicians and practitioners had filed an opt-out affidavit with CMS as of 2026-08-23, spread across 117 specialties and 55 states and territories.
Can I get a list of opted-out physicians by state or specialty?
Yes. The state filter runs server-side against the CMS API; specialty, keyword, ZIP prefix and effective-date-range filters are applied on the delivered rows.
Does this include only currently opted-out physicians, or expired ones too?
Both, by default, since that is what CMS publishes; 1,096 of 57,530 records had already expired as of 2026-08-23. Set currentlyOptedOutOnly to true to keep only active opt-outs.
